How to create your account
Walk through signup, email verification, and what to expect after. Works the same on either site.

Click Get Started
From the public site home page, click "Get Started" or open /quiz directly. The signup screen takes about 30 seconds.

Enter your email + password
Use the email you actually check — we send a verification link. Password is at least 6 characters.

Verify your email
Open the inbox, click the link in the verification email. The link expires in 15 minutes.

Complete the intake quiz
A short health-history questionnaire — under 5 minutes. Your responses go directly to your assigned clinician for review.
